Please fill in the following leave application at least 3 hours before your class, which relates to the following reasons ONLY : sick leave, vacation out of Hong Kong and school interviews. Unattended classes without leave application will be forfeited.
My Kiddy Gym will give you a call afterwards to help to book a make-up class for the unattended session. Please kindly hand in a proof of document within the make-up class.
